The evolving complexity of health care delivery requires medical professionals and public systems to adopt innovative, community-centered and data-informed approaches to care. The Advancing Community Care Conference, Unboxed: New Frontiers in Community Healthcare Delivery, offers a timely opportunity for physicians, clinical leaders and health care professionals to engage in meaningful dialogue focused on leadership, innovation and integrated models of care. The conference will take place on Wednesday, March 18, from 8 a.m. to 4:30 p.m. at the Rancho Cucamonga Events Center and offers five Continuing Medical Education (CME) credits.
