Make your mental health a priority with the AgeWise program. 

The San Bernardino County Department of Aging and Adult Services – Public Guardian is committed to helping older adults and individuals with disabilities maintain their independence, dignity and quality of life. Aging should be accompanied by respect, choice, quality of life and freedom, which are the heart of the Age Wise program.

Age Wise is a specialized behavioral health and wellness program for adults aged 59 and older. Aging may bring unique challenges such as changes in health, the loss of loved ones, feelings of isolation, or struggles with mental health and overall wellness. These experiences can feel overwhelming.

Age Wise is available to provide therapeutic and supportive services. The program provides compassionate behavioral health and wellness services, and connection to resources, such as food, assistance with utilities, affordable housing lists and more. The program’s goal is to help older adults live whole, meaningful lives rooted in recovery and resilience.

Trained clinicians, case managers and support staff work directly with older adults to provide therapy, counseling, resource linkage and support system development. Older adults are met whether they are in their home, via telehealth or in the office. Every individual’s story is unique and honored by creating personalized care plans tailored to their specific needs.

Age Wise’s mission is to ensure older adults in San Bernardino County have access to high-quality behavioral health and wellness services by focusing on practical support, professional care and building strong connections that enhance daily living.
